Patient Centered Medical Home Program

PCMH is the latest part of Arkansas Medicaid’s payment reform efforts intended to reward effective, evidence based care.

Similar to CPCI, the PCMH program is designed to improve population health, enhance the patient experience, and control the cost of care through an increased investment in primary care. PCMH builds on the efforts of CPCI by opening enrollment up to additional practices and offering some additional benefits, like data on your practice’s performance relative to peers, state, or national averages. Practices enrolled in PCMH are eligible to receive payments for care coordination, support for practice transformation, and shared savings incentives.

PCMH 2015 Pooling Enrollment
All PCMH providers are enrolled in a statewide pool for shared savings for the 2015 enrollment period. Providers have the option to pool voluntarily or standalone as long as beneficiaries total to 5,000. If you do not meet the 5,000 for six month requirement, you will not be eligible for shared savings. Check your email in the coming weeks for more information.
